To file a disability claim, of course you must be disabled, or believe that you are disabled.

You should immediately call your broker or agent to provide details and submit a claim. You can also contact the insurer directly.

A claims approver or claims specialist/underwriter will  review the specifics of the claim and notify you of their decision.

How to file a disability claim? Having done this myself for two policies a few years back, here's how it goes.

Contact the insurance provider and request claim forms be sent. Try to always communicate with one person at the carrier.
Paperwork will be quite a bit. Your doctors will have to sign off on your condition and offer their opinion of your situation. The insurer may request chart notes. This process will take some time. Contact the carrier and ask for paperwork a bit early. They may want to wait until you've met your waiting period. If you know you'll be there longer, just let them know and ask again for the paperwork. Remember, payments for disability claims come at the end of the month, not the beginning.
