Obtaining competitive disability coverage means you need to go shopping online. Long term disability insurance may be one of the most important insurance products you can purchase. Making sure you obtain a competitive quote means you're going to need to do a bit of Internet trolling. There are several brokerage outlets that represent some of the top disability companies in the country. Some of these outlets have comparison spreadsheets that can assist in determining the right policy for you.

To get disability insurance you simply find an insurance agent, choose a company, and apply.  It is important to note that some agents will work for one company and recommend that one company's product, while other agents (sometimes referred to as brokers depending on what state you live in) will represent several companies and can talk about benefits and draw backs of several insurance companies.

Once you apply you will need to provide financial documents supporting the claim that you actually earn the income you seek to protect and you'll need to undergo medical underwriting.  Medical underwriting typically includes a paramedical exam.  These exams include a nurse that takes your physical measurements and asks you a list of 20 or so health related questions.  In addition, medical underwriting will require a blood and urine sample to be collected and evaluated by a testing facility.   There is no cost to the applicant for any of these medical requirements as they are paid for by the insurer.
