There are several options to get health insurance while on disability. Your employer plan must be continued on the same basis as when you were working for up to twelve weeks if subject to FMLA. After that you can purchase the employer plan through COBRA.

You can purchase a policy through a state exchange outside of open enrollment if you qualify for a special open enrollment based upon your qualifying life event (loss of job qualifies). Or, you may have to wait for the annual open enrollment. You may qualify for premium and cost sharing subsidies based upon your lower income while disabled.

You may also qualify for Medicaid. The rules vary by state based upon whether your state adopted the ACA expansion.
