Your only hope of getting true major medical insurance for a family of 4 and keeping your costs under $100 per month would be through the government health insurance marketplace and being at an income level that would qualify you for a very substantial advanced premium tax subsidy.  Because Obamacare compliant health insurance plans are age rated I can't do an exact calculation without everyone in your family's date of birth.  But, I would estimate that your family's Modified Adjusted Gross Income for 2014 would have to be below $35,000 for you to get into the net cost range you are looking for.

Get health insurance for under $100 per month. Get health insurance coverage for $1 per month! Is it possible?

Thanks to the Affordable Care Act legislation, if you qualify for a federal subsidy, it is indeed possible to find a plan that has rates completely offset and paid by the subsidy.
