    It would be based on your family size, income and tobacco use. Base on your personal family composition, that would be use to determine if you qualify for subsidy or not . If you do qualify for subsidy you would subtract the amount of subsidy you are awarded from the total monthly premium, what's left is what you would be required to pay.
