    Assuming the question is addressing an underwriting procedure, most major medical plans do not request blood, urine or body fluid specimens for testing. An exception would be for individuals who have a "flag" in their file for previous underwriting discrepancies such as illicit drug, alcohol or tobacco use.

    Another example is someone who had a history of high cholesterol counts and could not produce recent numbers.
