It depends on the size of the policy.  On average it would be a paramedical exam, blood pressure, height weight, health questions, and blood, urine or saliva depending on the company.  A larger policy may require a full doctors physical with a stress test and the blood and urine.

Physicals for life insurance depend on the amount of coverage you're requesting and your age. Simplified issue may only require a battery of questions with a limited death benefit amount. But fully underwritten life insurance policies generally require a para med service that comes to your location, measures your height and weight, draws blood, collect a urine sample and sometimes an EKG. There could be additional exams if the amount or age or the proposed insured's history triggers further due diligence.

Keep in mind that all your medical records from all physicians will also be required with any prescriptions you're taking. They will order your driving record as well, looking for excessive traffic violations and any DUIs.
