Medigap or medical Supplement insurance covers generally any expenses that your Medicare insurance coves. Some crossover plans under Medigap policy means that the company is setup with Medicare insurance, so no extra paper work is needed just hand the medical provider both the Medicare and Medigap card. Some Medigap policy will pay some amount of Medical expense outside of the United States as Medicare will not. Usually $50000 max

In short, Medigap does not cover non-medically necessary procedures (e.g. cosmetic surgery) and items such as routine dental work and eye exams. Individual dental insurance plans and vision insurance plans will cover the latter programs - elective cosmetic surgery is generally going to be paid 100% out-of-pocket no matter what.

Medigap plans pay benefits when you have a Medicare-approved service claim.

Please note, each Medigap plan features different benefits so the question can expand from there as not every Medigap plan covers every available Medigap benefit. For example, plan F covers each Medigap benefit at 100% from day 1 (and 80% for foreign travel emergency care, up to plan lifetime limits of $50,000). Plan N covers each Medigap benefit at 100% from day 1 except for coverage of the Medicare Part B deductible ($147 in 2014 and 2015), Part B excess charges, and you have to pay a co-pay when you go to the doctor (up to $20 co-pay) or hospital (up to $50 co-pay).

I hope the information is helpful - please feel free to contact me for further help, including quotes and plan comparisons, and if you have any other questions. Thanks very much.
