When you are eligible at age 65 you will automatically be enrolled, you do not usually have to apply.  The initial enrollment period would be three months before, the month of, and three months after your 65th birthday or if disabled, your medicare eligibility date.  If you do not receive your Medicare card by your 65th birthday call your local Social Security office or 1-800-MEDICARE.
