    Medicare does not pay for refraction because they consider it a routine service, even if the refractive state of the eyes was ascertained for medical purposes rather than to prescribe glasses. Optometrists and ophthalmologists must charge refraction charges as a separate item so they are not accused of Medicare fraud.
