IVF (in vitro fertilization) is not a covered medical expense with most health insurance plans. Some employer group plans may include IVF benefits and there are a few states that mandate IVF coverage for individual major medical plans.

IVF is not a medical necessity which is why it is normally excluded from plan benefits.

The few times I have seen IVF benefits they are usually capped around $10,000 (lifetime) which means any excess charges are the responsibility of the individual.
