Typically, it is approximately $500-$5,000 (Yes...that is a big range) and it applied to major medical claims and often ER visits, inpatient surgeries and outpatient procedures. Usually, depending on the policy office visits and many prescriptions are not subject to the deductible. Of course, a higher deductible will lower your rates.
