To collect Disability insurance, you must have a disability that has lasted beyond the elimination period on your policy. E.g if you broke your back 2 months ago and your elimination period is 90 days, you will need to wait a little longer before collecting payments. It must also meet the definition of disability set forth in your policy.

Your claim form should explain your disability thoroughly. Make sure you have seen a doctor who has documented your illness or injury, and see a specialist if you can. Write a cover and date it so that you have a paper trail to go back to if your claim is denied.
