    When you sign up for a health insurance plan and it's approved, you should be sent a policy containing the details about your specific plan including the carrier name, plan name, deductible, monthly premium, coinsurance and copayment rates for various services, etc. The coverage is effective when the premium has been submitted to the carrier.
