    A health insurance 1500 claim form is a standard claim form issued by the Center for Medicaid and Medicare Services that used by a non-institutional providers and suppliers to bill Medicare insurance companies and durable medical equipment regional carriers, as well as some Medicaid State Agencies.  This form is used when a provider qualifies for a waiver from the Administrative Simplification Compliance Act requirement for electronic submission of claims.
