    Generally when we are talking about full-insured group health insurance plans they are regulated by the state insurance department of the state the group health insurance contract was issued in, this is usually the state where the company is headquartered.

    The situation, depending upon the circumstances, can get more complicated in the case of self-insured group health insurance plans. These plans are subject to the Federal ERISA law and are overseen by the Department of Labor.
