    A Home Insurance declarations page also known as the 'dec page' is the cover page or summary to the limits of coverage afforded on your specific insurance policy.   This page will include the Named Insured, Mailing Address, Property Address, Policy Number, Effective and Expiration Dates, Customer Service Contact Telephone Number, Claim Service Contact Telephone Number and Limits of Coverage.

    In addition, the declarations page usually includes the Policy Form Number which is the main policy form usually written by the ISO (Insurance Services Office) including all definitions, conditions and exclusions along with all the including endorsements that may be included with the policy. 

    If there is a Mortgagee listed on the policy,  the Mortgagee Clause will also be listed with the Name, Address & Loan Number of the mortgagee where a copy of the policy and all communication will also be directed.

    A declarations page is a summary of insurance coverage. It will list the effective date, coverage amount, premium, named insured & mailing address. It will also list the forms that are attached to the policy by form number.
