    Sometimes life insurance policies get lost.  If you have something with the name of the company, you can call the company and give them as much information as you can about the insured, social security number is very helpful, often they can find the policy in there files and send a duplicate.  If you have the policy number then that is even better.  If the company has been purchased by another company you may have to do some research or call the department of insurance to find out who purchased the company.
