    I assume you have lost the pieces of paper.  If that is the case, see if you can find a recent premium notice or check to see if money has been withdrawn to pay a premium.  Those items frequently have the policy number.  If you cannot locate the policy that way just write to the insurance company and tell them that a Billy goat ate your policy.  They will be glad to give you a copy.

    Finding a lost life insurance policy can take a little detective work.  If you know the name of the carrier, simply call and request either a copy of the policy (for a fee) or ask for the "declaration page" which will give you all the information you need free of charge.  If you don't know the company, check for past payments that were made through a checking account or other bank account.  Finally, check with findyourpolicy.com which may have a record of the policy.
