    To change life insurance ownership, you can contact the customer service department of the company through which your policy was purchased, and ask for a change of ownership form to be mailed, faxed, or emailed to you. You must sign the form and return it to authorize the change. Your agent who sold you the policy may assist you, if you ask them.
