You can sign up to receive Medicare benefits on the social security administration website, ssa.gov. I believe you can click on "Medicare" under the benefits tab. There should be some easy instructions there that will walk you through the sign up process.
Signing up online is preferable for those applying during their initial enrollment period (3 months before age 65, the month you turn 65, and the 3 months after you turn age 65). If you're applying under special enrollment, I recommend going down to your local social security office to get your paperwork processed.
