    If you are asking about how do insurance companies bill medigap policies, then it's generally either thru bank draft or quarterly, annual, or semi annual billing by mail. Some carriers still do monthly payment via a coupon book, but most are moving to bank draft. It offers the easiest processing for them and prevents the insured for worrying if a payment was sent in this month or not.
