   Congress created Medicare, a social insurance program, under the Social Security Act in 1965 to provide health insurance to individuals age 65 and up, regardless of income and medical history - later including those with disabilities who are under 65 or have end-stage renal disease.  Medicare is adminsitered by the Center for Medicare and Medicaid Services ("CMS" for short).
