There must be a health problem associated with being obese, such as sleep apnea or type 2 diabetes. The condition of being overweight  must have existed for at least 5 years. You must pass a psychological evaluation, have tried at least one other weight loss program that has failed, and other illnesses as the cause of your obesity must be ruled out.
