    Medicare offers very limited skilled nursing benefits for care after discharge from a hospital. Medicare does not cover any custodial or intermediate care at home, or care in a long term care facility.   To qualify for Medicare Benefits you must have been admitted to the hospital at least three days not counting date of discharge, and need skilled care related to the reason you were admitted to the hospital.  Medicare will not cover more than 100 days in a facility and will only provide limited benefits at home.
