    A Declaration Page, also referred to as a "Dec Page," tells you the most pertinent parts of your policy.  Along with containing the personal information (name, dob, address, etc.), it also lists your policy number, effective date, summarizes the coverage amount and states the premium.  Any changes (endorsements) or discounts will also be listed on this page.  The dec page also provides information on the insurance company, including how to contact them, where to call for claims, etc.
